This portal is for patients who have visited Bone and Joint Institute of Tennessee.

To obtain access to your child’s records:
To obtain access to your child’s records through the portal, complete one of the appropriate proxy forms below. (At age 18, proxy access to your child’s records will not be available through the portal.)
Please select from the following options:
I am a parent of a minor child age 0-13, setting up a portal account.
I am a parent of a minor child age 14-17, setting up a portal account.
I am an adult setting up a portal account for an adult patient other than myself.
